As Department Chaplain, it is my solemn duty to send memorial cards to the next of kin and record the passing of our departed Sisters in the permanent Department Memorial book. I must receive notification of the death of a Sister within 60 days or I am unable to send the memorial card. However, the name of the Sister will be entered in the Memorial Book.

It is the responsibility of the Auxiliary Graves Registrar to complete the Graves Registration form and mail it to me in a timely manner. Please type or clearly print the information to ensure correct spelling. Please try to complete the form in its entirety including the full name and address of the next kin as this information is needed to complete our final tribute. If the Sister you are reporting is a Life Member you must use the Member Change Request Form included with the Treasurer’s Kit to report her death to National Headquarters.
