Diane Pencak, Chaplain As Graves Registration Chairman it is my solemn duty to record the deaths of our Auxiliary Sisters. I will send a memorial card to the next of kin and record the deceased Sister’s name in the permanent Graves Registration Book. Please use the GRAVES REGISTRATION FORM which clearly indicates the four sections of the form: · The Auxiliary Chaplain’s Copy · The District Chaplain’s Copy · The Department Chaplain’s Copy · The National/Secretary/Treasurer’s Copy It is your responsibility to complete the Graves Registration Form and mail it to the appropriate person in a timely manner. Please try to complete the form in its entirety including the full name and address of the next of kin as all this information is needed to complete the final tribute. If the deceased Sister was a Life Member you must also use the Member Change Request Form included in the Treasurer’s Kit to report the death to National Headquarters. It is very important that I receive the form not later than 60 days after the death of a Sister. If a form is received after 60 days no Memorial card will be sent, however, her name will be placed in the Graves Registration Book. Please review the Graves Registration Program and Form at the links provided.
